The Texas Department of State Health Services has established minimum immunization requirements for all students in public and private schools. Nazareth Academy, as well as all schools in the diocese, adheres to the State immunization requirements.
The school must have a copy of your child’s/children’s immunization record proving compliance, BEFORE the first day of school. All immunization records must include: day, month and year, and be validated by a doctor’s signature or a clinic stamp. A copy of a previous school immunization record is not acceptable.
Your child will not be allowed to attend school until proof that the requirements have been met. Please bring, send, or fax a copy of the updated immunization record to the school
